This form is for 9th Grade students ONLY.

Please fill out the form below to request pre-approval by administration for an extended absence.

Please note: A medical appointment or short-term illness is not an extended absence and will still count toward the 4 absences allowed per quarter. If the absence is due to a long-term illness, please explain above.

For K-8 students, please fill out the Excuse Absence K-8 form.
